1. Initial Phone Conversation

This is where we see if we are a good fit for one another.  I want you to be as comfortable with your decision to work with me, as I want to be with you.  You will have the opportunity to get any questions answered and it will give me the opportunity to learn more about you, your health goals and your expectations.

2. Submit Intake Form and Schedule Appointment

At the conclusion of the initial consult, download and complete the Health Intake Form here.  (You must have Adobe Reader, available free from Adobe.) Please be sure to include which package you are interested in as well as payment information for laboratory services. Please fax the completed forms to
734-468-8573.

After submitting your intake form, please call 443-867-7619 to schedule your appointment and verify that your fax was received.

3. Complete Laboratory Testing

Within one business day you will receive an email with a blood chemistry requisition form. Print out the form and bring it with you to your nearest LabCorp location for a fasting blood draw. These results will be reviewed during your appointment.
